Wellness CoachThe Wellness Coach supports patient wellness and health integration in outpatient clinics. Wellness coaches are an integral part of a patient's Care Team and work within multidisciplinary teams and other support staff. This position is responsible for engagement of clinic patients into a variety of healthcare activities through population-based assessments and screenings and tracking patients' follow-up and progress.
